Even if I have showered prior to arrival, I will shower with soap, wash my hair, and not enter the float pod with any oils or creams on my body.  *

I understand that if I have a keratin hair treatment, it can be affected by any saltwater, especially the highly saline water in the float pool.*

I do not have any communicable disease (COVID, Chicken pox, HIV, Tuberculosis, measles, etc.).*

I am not under the influence of drugs and/or alcohol.*

I understand that if I am epileptic, I have consulted with my physician and my epilepsy is under medical control. I am in sufficient control of my seizures not to endanger my floating experience. *

I understand that if I am pregnant, I have consulted and received permission from my physician to use the floatation pool.*

I understand that if I suffer from diabetes, that I have consulted with my physician and have been cleared by my physician, to use the floatation pool.*

I understand that if I suffer or have suffered from chronic heart disease, I have consulted with my physician, and have been cleared by my physician, to use the floatation pool.*

I understand that if I suffer or possess any sort of existing ear, nose or eye condition, that it may be irritated by properties within the floatation pool.*

I do not have a condition, nor am I on any medication which may have adverse effects to the immersion in the concentrated magnesium sulfate (Epsom Salt) water solution.*

If I have any major medical problems such as epilepsy, kidney disease, low blood pressure, or gastroenteritis, I have consulted with my physician and obtained permission to participate in flotation therapy which involves the immersion in concentrated magnesium sulfate.*

I do not have any open wounds or skin ulcers.*

I certify that I am not under any type or medical restrictions by my physician.*

I understand that floating may lower blood pressure, and I will take extra care standing up after my float.*

If I have a history of (> = 180/120) or low (<=90/50) blood pressure, I have medical authorization to use the floatation pool.*

I understand that if I suffer from vertigo when lying down, the same could occur during a float session.*

I have not shaved or waxed 48 hours before my floating experience, and if I have, I understand that there may be some irritation.*

I have not used self-tanning products in the last 48 hours prior to floating.*

I am physically capable of getting in and out of the float pod on my own.*

I understand that the flotation pool uses Epsom salt, natural enzymes, botanical extracts, and non-toxic, biodegradable cleaning products may cause skin irritation, allergies, or adverse skin reactions.*

I have not eaten a heavy meal in the last 2 hours.*

I have not had diarrhea in the past 48 hours.*

I am not menstruating.*

I understand that no bodily fluids, blood, stools, or liquids of any kind are allowed in the pool. If I put any fluids or liquids of any kind in the pool it is considered contamination in the float pool.*

I acknowledge that if I contaminate the floatation pool in any way, I will be required to pay a contamination fee which may include (cost of cleanup, refilling the pool and lost revenue of scheduled appointments for the next 24 hrs. The contamination fee may range from ($500 to $2,000).*

I acknowledge that failure to pay the contamination fee may result in me being permanently banned from the use of Relax and Restore Float Spa.*

I acknowledge I will be responsible for all legal fees required to collect the contamination fee.*

I will remove all jewelry and turn off all electronic devices before entering the float pool area. I acknowledge that any jewelry that I leave on or electronic devices that I take into the float room will be at my own risk.*

The flotation pool is used for relaxation purposes and needs to remain a quiet, tranquil environment. Please make every effort to be respectful while floating. splashing kicking, talking or other disruptive behavior is not allowed.*

I am 18 years of age or older.*

All float pools are in wet areas, and I will take extra precautions for my own safety.*

Upon using the flotation pool, I am choosing to use the flotation pool of my own free will. I agree not to hold Relax and Restore Float Spa operators, or owners liable for any injury to self or for loss or damage of personal items directly or indirectly.*

By signing below, I have read and understand the above statements and agree to comply with the rules of this facility.*
